Spiders - No matter the size, I just can't handle them. I can trace this back to playing Sim Ant at age 6. When you, the ant, got caught by the spider, there would be a close-up on the screen of the spider eating you and it terrified me. I can't stand the things anymore.
Stinging insects - I was stung in the hand by a wasp when I was 9 or 10. I remember it hurting terribly so now they scare me. I have a feeling it wasn't nearly as bad as I thought it was at the time, but that's what happens when you're a kid, I suppose.
Snakes - They just give me the heebie jeebies. Ick.
Heights - I can fly and ride roller coasters without a problem. But getting on ladders, hiking mountains, escalators, etc terrify me. I suppose I'm more afraid of losing my balance and falling a long way than anything. With airplanes and roller coasters, there isn't anything to worry about with my balance because you're strapped into your seats. The others involve the chance you could slip and fall. That's what scares me.
Enclosed spaces - I trace this one back to riding an enclosed, completely dark water slide at a water park when I was younger. I'd never had a problem with tight spaces before, but that water slide terrified me that I might get stuck and be unable to find my way out. Ever since, I can't do enclosed spaces. I had an MRI on my knee a few years ago in one of the tubes; I only went about halfway in but I was close to freaking out. Completely illogical, but I can't help it.
Driving over bridges - I'm more afraid of the bridge collapsing and not being able to get out of my car before it sinks... seeing bridge collapses on the news doesn't help.
